In this grand rounds lecture Sarah Hallberg, DO, outlines recent research that suggests patients with metabolic diseases like type 1 and type 2 diabetes can achieve exceptional glycemic control using a low-carb ketogenic diet. She summarizes the current dietary recommendation for patients with metabolic disease and why this approach may not be effective, and discusses ways to help support patients in the long-term while maintaining this type of diet. Dr. Hallberg is the Medical Director at Virta Health and Medical Director and Founder of the medically supervised weight loss program at Indiana University Health Arnett.

Thank you doctor. This is the first video that I've seen explaining what I've been telling people for years. I started an atkins diet 30 years ago and allow myself 12 carbs per day but I don't eat a keto diet. I actually don't keep up with how much protein or fats I consume although I do make an effort to cook with butter, eat eggs and some fatty meat, cheese, nuts and seeds. I likely stay in ketosis but I don't follow the limitations of a keto diet. Unfortunately keto has become the buzz word for all things low carb and it's used wrong. There are no keto recipes, no keto foods. They are all low carb items that are acceptable on a keto diet. Looking around many things are now labeled keto but they by themselves are only low carb. I've told people for years keto is low carb but low carb is rarely keto even if you're in ketosis. That seems to cause the most confusion among people. A keto diet is a strict diet where you have to remain in the guidelines for it. It doesn't matter if you're in ketosis if you're not following the keto guidelines. You're simply eating a low carb diet. There are multiple low carb diets that will get you into ketosis but as far as I'm aware only one keto diet exists. I also have noticed that many people are insisting that 50 to 100 carbs per day is low carb. I'll go by atkins. Start below 20 per day. After you get to the maintenance phase you can add 5 carbs every couple of weeks testing until you drop out of ketosis and then you have to reduce carbs some. The majority of people will likely fall somewhere between 20 to 30 carbs unless they are very physically active. Always have blood work done at least twice a year to check for issues. My doctor had me increase salt and decrease water intake to under 50 oz per day to maintain the correct sodium levels. One last thing that needs to be made clear to new low carb dieters. You don't control weight loss by low carb, you do it by reducing calories you consume. Low carb allows you to burn fats and also reduces your appetite making it easier to eat less.

I've been trying to find out the benefits of a low carb diet. I am not overweight nor diabetic. A low carb diet is described in this video but not really talked about.

Doing a 2 month clean Keto last year changed everything for me since it made me change the taste buds and not crave sugar anymore and get into fasting. It made me love the darkest kind of chocolates and good olive oil etc and even though I cheat from the diet a lot these days the sugar craving hasn't returned and my health has improved so much it's ridiculous. So anyone wanting to say cut down on sugar the best is to remove all carbs cause if you just start just cutting down you'll still have the cravings which on full keto are gone in a week or two and then it's super simple to stay like that instead of this constant struggle with discipline I see many go through

There's no such thing as a sub species nor did we evolve around our diet…we simply gained already but unknown technology which allowed us to manufactured foods which aren't as healthy as the food and it the state which God gave it to us

According to my macros, I’m definitely Keto, been on this diet for 4 weeks. Except I don’t seem to be in ketosis, urine strips only show a trace. Plus, I’m barely losing weight. Could insulin resistance be affecting this? Diet soda?

What’s been great on LCHF is not only losing 35 lbs in the past five months…I’m also able to exercise six times/wk without injury and have more energy. I do anywhere from 75-150 carbs/day, and no added sugars or processed foods. Life-changing!

I went through diabetic ketoacidosis a few years ago and was about a day away from going into a diabetic coma. I was in the ICU for 2 days. I wouldn’t recommend it.

Question: I started low carb 3 days ago and eat 50 or less carbs a day. Is this low-carb or keto?
November 4th, 2023 at 16:01
It is unfortunate you suggest we follow GOVERNMENT guidelines as they have changed, in some cases 360degrees from the first suggestion. Eggs were bad, then good , then bad, then ok in moderation , then good again.
Please for the future, do not suggest the government is up to speed with what's best and focus on your real research. Thank you

Big pharma doesn’t like people being in ketosis because they make too much money on complications from obesity.

will the low carb diet be more efficient in terms of gaining muscles?
November 4th, 2023 at 17:55
Doc. I like your definition. A keto diet is a diet that puts you in ketosis regardless of the food. One thing to note is that protein produces an insulin response. Even on a zero carb diet, consuming very lean protein can take someone out of ketosis (I’m proof of that).

What is a Ketogenic Diet and Glycemic Diet I don't know ! I have had a Sister and 2 Brothers that have died from diabetes

Still didn’t get to the ROOT cause of insulin resistance. It’s bc too much fat is stored in places it shy be, like skeletal muscle, etc, which disrupts the way insulin can work at the level of the cell. Going keto just removes the sugar, great, but only going low fat actually gets to the root of the problem of insulin resistance.

What fruits should diabetics avoid?
November 4th, 2023 at 22:52
One reason why exercise seems to help with diabetes type 2 is that it uses energy. Insulin is also an energy storing hormone.
So, it seems that fasting would help diabetes type 2 and insulin in using energy so the body becomes more insulin sensitive.
One problem with fasting is that it can be done wrong. One's body should be fat adapted first to avoid some issues with fasting. Then, one can fast to have lower insulin levels, while keeping fasting more sustainable and safer. Then, the body can use its stored energy and become more insulin sensitive.

How do you prevent T2D patients from going into ketoacidosis? Is the UI ketogenic treatment program basically a gradual weening of carbohydrates with increasing intake of fats & protein through continuing monitoring?
November 5th, 2023 at 02:14
Thank you Dr. Hallberg. Hope you're resting well in heaven.
November 5th, 2023 at 02:32

Thank you for information.

Dr Hallberg was a true blessing! Imagine dedicating you life and knowledge to helping people! How unfair it is her dying from cancer? So many people in the world that have no desire or passion to help humans and they live long lives? She touched so many people, and its terribly sad to watch her so on fire and sharp as a tack, and a few years later gone? Life is marvelous, but surely unfair! Imagine if she lived, she would have had a whole lifetime of more strong work! A great loss! I think of singer Eva Cassidy that died in 2005 ! How s life so unfair- forgive me,we have a huge part of population that will never educate themselves, and/or dedicate thier lives to do what she did! She is so succinct in her delivery ! She is a true communicator! Darn it!

Why so many RIPs? People who die are gone from this world and are not resting anywhere. Sarah was a marvellous dietician and, like many others, helped me so much in my journey to rid myself of diabetes meds.
